With your arm bent 90 degrees at the elbow, the natural position is with your thumb pointing up. The cordless feature is actually nice--it lets you use the mouse on your lap, on the couch, on the armrest, etc The wireless range is pretty good too--I can use it in the next room over why? This can get a bit annoying when using the scroll wheel, since your middle finger is left sitting on top of the ball, with your index finger on wheel on the left and your ring finger on the right-click button to the right the stretch is uncomfortable for long periods of time. The spacing is a bit awkward, but really that is my only complaint. For most cases, I prefer to use my keyboard to scroll down pages, because it puts a lot less wear on my poor Logitech Cordless Optical TrackMan wrist, so I don't have to deal with this issue much.

I haven't tried trackballs designed to be controlled with your thumb, but trying to use this one with my thumb is very imprecise not a Logitech Cordless Optical TrackMan of the device, but rather Logitech Cordless Optical TrackMan can't imagine using my thumb to accurately and precisely move the cursor! As a side note--I'm interested in a comfortable device for constant slow scrolling through pages because keeping my hand on the mouse or keyboard for those times, while reclining, gets uncomfortable. I was thinking of repurposing an old mouse scroll wheel. Anyone know of any existing solutions?
Anyways, if you haven't used a trackball before Not only did it almost completely relieve my wrist pain, but I also found that it is totally superior for high-precision applications such as CAD. It is loaded Logitech Cordless Optical TrackMan buttons, seems to have a very ergonomic shape, and it is wireless. But when I started using it, I quickly find out that it has some drawbacks.
It takes some serious practice to get a hold of this one. This is a difficult trackball, and you need to adjust the way you use mice or trackballs to be able to use it properly.
Once you have dedicated Logitech Cordless Optical TrackMan intense training time with this device, you might have found a very good new friend. When I take the Logitech Cordless Optical Trackman out of the box, I admire the shape and soft material of the handrest.

It has a very luxurious feel to it, and the trackball is solid and well made. There are two models available, one as pictured above with the dark grey plastic, but another version is available with silver grey Logitech Cordless Optical TrackMan.
The darker version looks much nicer in my opinion. I first have to insert the two batteries which are included in the box and pair the device with the wireless receiver, simultaneously pressing a button on each device. The button on the Trackball is one of those tiny buttons you have to press with a pen or paperclip.
